Title: Performance Specialist

Degree Type/Area: Bachelor’s degree in Social Work or a related field. Master’s degree preferred.
Description:
The Performance Specialist is responsible for quantitative assessment of performance throughout the FSMO system of care. The Performance Specialist will be an expert FSFN user. The position will ensure the use of system data to focus on programmatic performance and make recommends for quality improvements.
Specific Responsibilities:
- FSFN liaison and “super user” for the lead agency and case management staff;
- Continuous monitoring of FSFN data integrity;
- Monitor and develop tracking tools for case management and contracted client services;
- Develop and provide analysis of service center and unit level performance scorecards;
- Assist in the development of improvement plans needed to rectify performance issues;
- Point of contact for case management staff for AFCARS and placement errors in FSFN;
- Perform ad hoc performance reporting of FSFN data through “Spinner Reports” and the FSFN Data Mart.
- Responsible to collect data and produce monthly Team Performance Call (TPC);
- Analysis of data and development of improvement plans;
- Point of contact for case management (CMO) Change Management Specialist;
- Facilitate monthly meeting with CMO Change Management Specialist;
- FSFN subject matter expert trainer, co-training with certified trainer.
