Title: Relative Navigator

Description:
This position will have the lead responsibility for providing support to all relatives who have children from the child welfare system placed with them. This will include training, direct technical support to the relatives and the Case Management Organizations (CMO) and community outreach. This position is based in Orange and Osceola County, reporting to the Medicaid Manager.
Specific Responsibilities:
- Serves as liaison for all Relative Caregiver communication forms sent from the CMO’s and ACCESS. When the forms are received, the relative and the child must be researched on FLORIDA, FMMIS and FSFN to determine the correct routing of the form. The forms must be logged into a spreadsheet and the hard copies must be retained. When action is taken on the request, the Relative Navigator must log the disposition of the request and route the completed form back to the sender.
- Assists the relatives with their ACCESS applications and continues to provide information and support to them after their case is approved.
- Provides technical assistance to the CMO’s with status reports on specific relatives’ benefits and answers general questions regarding ACCESS procedures and eligibility criteria.
- Designs and delivers training sessions for relatives who have recently had children placed with them.
- Prepares handouts and guides to distribute to relatives, case managers and other stakeholders.
- Attends case manager pre-service training and CMO meetings/in-services trainings to give overviews of the ACCESS Relative Caregiver process.
- Provides support and information to local relative support groups.
- Attends community events to disseminate information regarding benefits available to relatives and to network with other stakeholders.
- Act as a liaison between the relatives and the Area 7 Medicaid offices to resolve any managed care or billing issues.
Qualifications:
Education & Experience:
- Candidate must demonstrate attention to detail, organizational abilities, time management skills and computer application skills, including spreadsheet applications.
- Knowledge of FSFN, FLORIDA and FMMIS systems preferred
- Bi-lingual preferred. (Spanish)
- Knowledge of ACCESS policy and procedures required.
- Must have competencies in data management via hard copy and on a computer.
- Knowledge of HIPAA requirements is preferred.
- Must have a strong accountability and follow through on assignments, ability to multitask.
- Bachelor’s degree preferred;
- Two years experience in ACCESS and or child welfare.
